When the valves fail to close, blood flows backwards (reflux), the veins dilate and varicose veins appear.<\/p><p>It is a common condition: a review in the <a href=\"https:\/\/www.medigraphic.com\/pdfs\/revmexang\/an-2018\/an184f.pdf\" target=\"_blank\" rel=\"noopener\">Revista Mexicana de Angiolog\u00eda<\/a> estimates a prevalence of about 20% in Latin America. That is why it is worth telling apart a visible vein that is only a cosmetic concern from venous disease that needs treatment.<\/p>\n<h2 id=\"s2\">Signs that an assessment is worthwhile<\/h2><p>The IMSS guideline lists the symptoms that point to venous insufficiency. If they keep coming back, it makes sense to see a specialist:<\/p><div class=\"cols\"><div class=\"col no\"><h3>Symptoms<\/h3><ul><li>Heavy, tired legs, especially in the afternoon<\/li><li>Pain that worsens when standing or in the heat<\/li><li>Night cramps and itching<\/li><li>Ankle swelling that improves with rest<\/li><\/ul><\/div><div class=\"col no\"><h3>Skin signs<\/h3><ul><li>Dark patches on the lower leg<\/li><li>Hardened skin or eczema<\/li><li>Wounds that are slow to heal (venous ulcer)<\/li><li>Veins that bleed easily<\/li><\/ul><\/div><\/div><p class=\"note\">Skin changes and ulcers indicate more advanced disease. In these cases, the <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/35027279\/\" target=\"_blank\" rel=\"noopener\">ESVS 2022 guideline<\/a> recommends treating reflux early because it speeds healing and reduces recurrence.<\/p>\n<h2 id=\"s3\">What Doppler ultrasound checks<\/h2><p>Treatment is not decided by how the legs look. The <a href=\"https:\/\/www.imss.gob.mx\/sites\/all\/statics\/guiasclinicas\/175GRR.pdf\" target=\"_blank\" rel=\"noopener\">IMSS guideline<\/a> recommends <strong>duplex Doppler ultrasound<\/strong> to confirm the diagnosis in treatment candidates. The scan shows:<\/p><ul><li>Whether there is reflux and in which veins (great saphenous, small saphenous or branches).<\/li><li>The diameter and course of the vein, which determine whether the laser fiber fits.<\/li><li>Whether there is thrombosis or a problem in the deep veins.<\/li><\/ul><p>With that result, the specialist classifies the disease using the international <strong>CEAP<\/strong> scale, which the same guideline recommends.<\/p>\n<h2 id=\"s4\">The CEAP scale made simple<\/h2><p class=\"lead-a\">The CEAP scale grades venous disease from C0 to C6 according to what is seen on the leg.<\/p><table><thead><tr><th>Class<\/th><th>What is seen<\/th><th>What it usually means<\/th><\/tr><\/thead><tbody><tr><td>C0 to C1<\/td><td>No visible signs or spider veins<\/td><td>Assessment if there are symptoms; often general measures<\/td><\/tr><tr><td>C2<\/td><td>Varicose veins<\/td><td>Doppler to look for reflux; treatment if a candidate<\/td><\/tr><tr><td>C3<\/td><td>Swelling (edema)<\/td><td>Treat the venous cause when Doppler confirms it<\/td><\/tr><tr><td>C4<\/td><td>Skin changes<\/td><td>Higher treatment priority<\/td><\/tr><tr><td>C5 to C6<\/td><td>Healed or active ulcer<\/td><td>Early treatment of reflux according to ESVS<\/td><\/tr><\/tbody><\/table><p>Source: CEAP classification recommended by the <a href=\"https:\/\/www.imss.gob.mx\/sites\/all\/statics\/guiasclinicas\/175GRR.pdf\" target=\"_blank\" rel=\"noopener\">IMSS<\/a> and the <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/35027279\/\" target=\"_blank\" rel=\"noopener\">ESVS<\/a>.<\/p>\n<h2 id=\"s5\">When endovenous laser is the first choice<\/h2><p>The <a href=\"https:\/\/www.nice.org.uk\/guidance\/cg168\" target=\"_blank\" rel=\"noopener\">NICE guideline (CG168)<\/a> recommends this order for people with symptomatic varicose veins and confirmed reflux: first, endovenous thermal ablation (laser or radiofrequency); if unsuitable, ultrasound-guided foam sclerotherapy; and if that is also unsuitable, traditional surgery. Compression stockings are reserved for people who cannot receive treatment.<\/p><p>There is published experience in Mexico: a study of 158 limbs treated with endovenous laser reported mild complications in about 20% and no cases of deep vein thrombosis or embolism (<a href=\"https:\/\/www.scielo.org.mx\/scielo.php?script=sci_arttext&amp;pid=S2696-130X2023000300071\" target=\"_blank\" rel=\"noopener\">Revista Mexicana de Angiolog\u00eda, 2023<\/a>).<\/p>\n<h2 id=\"s6\">What if I wait?<\/h2><p>Venous disease is chronic. Waiting may be reasonable if there are no symptoms and no significant reflux, always with venous relief measures. But when there are persistent symptoms, swelling or skin changes, delaying the assessment lets the disease move into higher CEAP classes.
