Varicose veins are one of its most visible signs. In Latin America, a review in the <a href=\"https:\/\/www.medigraphic.com\/pdfs\/revmexang\/an-2018\/an184f.pdf\" target=\"_blank\" rel=\"noopener\">Revista Mexicana de Angiolog\u00eda<\/a> estimates a prevalence of about 20%.<\/p><p><strong>Symptoms described by IMSS:<\/strong> heaviness, leg pain that worsens in the afternoon, itching, night cramps, fatigue and swelling that improves with rest. They tend to worsen when standing or in the heat and improve when lying down or raising the legs.<\/p><p><strong>Risk factors:<\/strong> older age, family history, long periods standing, obesity, pregnancy, a sedentary lifestyle and certain occupations. The condition is more common in women.<\/p>\n<h2 id=\"s2\">What endovenous laser is and how it works<\/h2><p>Varicose veins appear when the valves in the leg veins stop closing properly and blood pools. In many patients the source is the great or small saphenous vein. In endovenous laser, the doctor inserts a thin fiber into the vein under ultrasound guidance. The laser energy heats the vein wall from the inside and closes it, and blood is naturally rerouted through healthy veins. That is why it is classified as <strong>endovenous thermal ablation<\/strong>, the same family as radiofrequency. The <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/35027279\/\" target=\"_blank\" rel=\"noopener\">ESVS 2022 guideline<\/a> confirms the central role of thermal ablation in treating superficial vein insufficiency.<\/p>\n<h2 id=\"s3\">How it is diagnosed before treatment<\/h2><p>The <a href=\"https:\/\/www.imss.gob.mx\/sites\/all\/statics\/guiasclinicas\/175GRR.pdf\" target=\"_blank\" rel=\"noopener\">IMSS guideline<\/a> recommends <strong>duplex Doppler ultrasound<\/strong> for accurate diagnosis in treatment candidates, and classifying the disease with the international <strong>CEAP<\/strong> scale, which considers clinical signs, cause, affected veins and mechanism. With that information the specialist decides whether the saphenous vein is suitable for laser, whether another technique is better, or whether conservative measures are enough. Another Mexican study of 80 patients comparing laser and radiofrequency found no significant differences at 7 and 30 days (<a href=\"https:\/\/www.scielo.org.mx\/scielo.php?script=sci_arttext&amp;pid=S2696-130X2021000400101\" target=\"_blank\" rel=\"noopener\">Rev Mex Angiol 2021<\/a>).<\/p>\n<h2 id=\"s6\">Endovenous laser compared with other options<\/h2><p class=\"lead-a\">Guidelines rank thermal ablation, such as endovenous laser, as the first choice; foam, surgery and stockings are kept for other cases.<\/p><ol><li><strong>Thermal ablation (laser or radiofrequency):<\/strong> first choice if the patient is a candidate (<a href=\"https:\/\/www.nice.org.uk\/guidance\/cg168\" target=\"_blank\" rel=\"noopener\">NICE CG168<\/a>, ESVS 2022).<\/li><li><strong>Ultrasound-guided foam sclerotherapy:<\/strong> an alternative when ablation is not suitable.<\/li><li><strong>Traditional surgery (stripping):<\/strong> when the above are not suitable.<\/li><li><strong>Compression stockings:<\/strong> only when no treatment is possible.<\/li><\/ol><p>Earlier versions of the <a href=\"https:\/\/cochranelibrary.com\/cdsr\/doi\/10.1002\/14651858.CD005624.pub4\/information\" target=\"_blank\" rel=\"noopener\">Cochrane review<\/a> concluded that laser, radiofrequency and foam are at least as effective as surgery, and in the <a href=\"https:\/\/evidence.nihr.ac.uk\/alert\/laser-treatment-is-the-preferred-treatment-for-troublesome-varicose-veins\/\" target=\"_blank\" rel=\"noopener\">CLASS trial<\/a> recovery with laser was faster.<\/p>\n<h2 id=\"s7\">Recovery step by step<\/h2><p class=\"lead-a\">Patients walk the same day and most return to their activities within a few days.<\/p><ol><li><strong>Same day:<\/strong> the patient walks out; bandage or compression stocking as indicated.<\/li><li><strong>First days:<\/strong> mild discomfort and bruising; walking is encouraged and long sitting avoided.<\/li><li><strong>First weeks:<\/strong> control ultrasound to confirm closure and rule out thrombosis.<\/li><li><strong>Following weeks:<\/strong> visible varicose veins shrink; some branches may need complementary treatment.<\/li><\/ol><p class=\"note\">Seek immediate care for severe pain or sudden leg swelling, shortness of breath or chest pain.<\/p>\n<h2 id=\"s8\">How to care for your veins before and after<\/h2><p>The <a href=\"https:\/\/www.imss.gob.mx\/sites\/all\/statics\/guiasclinicas\/175GRR.pdf\" target=\"_blank\" rel=\"noopener\">IMSS guideline<\/a> recommends regular physical activity (especially walking and swimming), calf exercises when sitting for more than 5 hours, avoiding constipation and consulting a doctor before starting hormone therapy.
