Doctor explaining options to restore breast firmness with an anatomical model
Summary

During pregnancy and breastfeeding the breasts grow and then shrink; that change stretches the skin and supporting tissue, and it is common for the breast to sag or lose volume. Exercise, creams or garments do not lift the bust, because sagging depends on the skin and internal support. When sagging is real, the medical option is a breast lift, alone or with implants if volume was also lost. It is best to wait until breastfeeding ends and weight is stable before deciding.

Why the bust changes after pregnancy

During pregnancy the breast gland grows to prepare for breastfeeding; afterwards it shrinks. That cycle of growth and reduction stretches the skin and ligaments that support the breast. The American Society of Plastic Surgeons lists pregnancy, breastfeeding, weight changes, age and gravity as causes of lost breast firmness.

Results vary widely: some women nearly recover their previous shape while others notice sagging, loss of volume in the upper breast or larger areolas.

Common changes

Shape changes

  • Sagging (ptosis)
  • Loss of volume in the upper pole
  • Asymmetry between the breasts

Skin changes

  • Looser skin
  • Stretch marks
  • Wider areolas

When it is worth waiting to decide

  • Finish breastfeeding and allow a few months for the bust to settle.
  • Reach a stable weight, because weight changes alter the result.
  • Not planning another pregnancy soon, as a new pregnancy can change the shape again.

Deciding once the bust is stable helps the surgeon assess the real degree of sagging and choose the right technique.

Which options exist and for whom

SituationUsual optionWhat it achieves
Sagging with enough volumeBreast liftLifts, firms and repositions the areola
Sagging and volume lossLift with implantsLifts and restores volume
Volume loss only, no saggingConsider augmentationVolume; does not correct sagging

Sagging is measured with the Regnault classification, according to the nipple position relative to the fold under the breast.

What does not lift the bust

Chest exercises strengthen the muscle behind the breast but do not lift the gland or the skin. Creams and garments do not correct sagging either. They may have health value, but they do not replace surgery when sagging is real.

What Mexican evidence says about results

In a study of 149 patients by plastic surgeons in Mexico City, a lift alone had the highest satisfaction (75 of 75), above lift with augmentation (16 of 26) and implant exchange (17 of 48) (Cirugía Plástica, 2022). In other words: an implant is not always needed; it depends on whether volume was lost.

How to choose where to get care

COFEPRIS recommends checking that the surgeon is registered with the Mexican Council of Plastic, Aesthetic and Reconstructive Surgery and confirming their training.

Myth and fact

Myth: “Exercise lifts sagging breasts.”

Fact: It strengthens the muscle but does not raise the gland or the skin.

Myth: “Breastfeeding is the only cause of sagging.”

Fact: Pregnancy, weight, age and genetics also play a role (ASPS).

Myth: “Implants are always needed.”

Fact: Only if volume was lost; a lift alone had the highest satisfaction in the Mexican study.

Myth: “Surgery can be done while breastfeeding.”

Fact: It is best to wait until it ends and the bust settles.

Frequently asked questions

How long after breastfeeding can I have surgery?

It is advisable to wait a few months after finishing, until the bust settles; the surgeon decides.

Does the bust return to its shape on its own?

For some women largely yes; for others sagging remains that cannot be corrected without surgery.

Lift or implants?

A lift for sagging; implants for lost volume; both if there is both.

Can I get pregnant again afterwards?

Yes, but it can change the result; that is why surgery is best after family planning.

Do firming creams work?

They do not lift sagging breasts.

How do I know my degree of sagging?
