In short

A laser buttock lift aims to lift and shape the buttocks with the patient’s own fat: it is removed by laser-assisted liposuction from areas such as the abdomen or flanks and grafted into the buttock. It is a 2 to 3 hour surgical procedure. The key to its safety is injecting fat only under the skin and never into the muscle, as plastic surgery societies have recommended since 2017. With these measures, reported mortality dropped from 1 in 3,448 to 1 in 14,952 cases.

Quick facts
Who it is for
Sagging or lack of buttock projection, with good general health (Gonzalez 2006)
Duration
About 2 to 3 hours
Anesthesia
Defined by the anesthesiologist for the procedure (ASPS)
Recovery
Weeks, avoiding direct pressure on the buttocks (ASPS)
Results
Minor complications in 3.58 % across 22,151 patients (Plast Reconstr Surg 2026)
Diagnosis

Why do buttocks sag?

Buttocks lose firmness and position with weight gain, age and weight loss.

A study in Aesthetic Plastic Surgery (2006) proposed classifying sagging as pseudoptosis, preptosis and true ptosis, with five severity grades, and found that weight, age and weight loss are related to severity.

Common signs

  • Longer or deeper fold under the buttock
  • Loss of projection or a flat shape
  • Laxity after weight loss
  • Changes with age
Procedure

How is it done?

It is done in four stages, from assessment to follow-up.

Assessment

Degree of sagging, fat donor areas, general health and expectations.

Laser liposuction

Fat is obtained from areas such as the abdomen or flanks with laser support.

Grafting under the skin

Fat is placed only in the subcutaneous plane, never into the muscle.

Follow-up

Check-ups, compression garment and positioning care.

Candidates

Who is it for?

It is indicated for people with sagging or lack of projection, enough fat to graft and good general health.

Usually a candidate

  • Sagging or lack of buttock projection
  • Enough fat in donor areas
  • Stable weight and good health
  • Realistic expectations

Needs special assessment

  • Very little fat available
  • Uncontrolled medical conditions
  • History of modeling substances in the buttocks (COFEPRIS)
  • Ongoing weight changes
Scientific evidence

What do studies say about its safety?

Gluteal fat transfer is safe when the plane under the skin is respected; studies confirm it with figures.

The same meta-analysis found fewer major complications with ultrasound guidance (0.02 % vs 0.08 %) and the lowest rates with subcutaneous-only injection.

Myth vs fact

What is myth and what is fact?

“The deeper the fat is injected, the better.”

It is the opposite: injecting into the muscle raises the risk of embolism; only under the skin is recommended (ASERF 2017).

“All grafted fat stays.”

Part of the fat is reabsorbed, which is why the result is assessed over the following months (ASPS).

“It is the same as biopolymer injections.”

No. Your own fat is your tissue; biopolymers are high-risk substances (COFEPRIS).

“It is a risk-free procedure.”

It has risks, mostly minor, that are reduced with the right technique (2026 meta-analysis).

FAQ

Questions about the buttock lift

What is a buttock lift?

A procedure to lift and shape the buttocks; at the clinic it is done with the patient’s own fat and laser technology support.

Are implants used?

No. The patient’s own fat, obtained by liposuction, is used.

How long does it take?

About 2 to 3 hours.

Is it safe?

With injection only under the skin, reported mortality dropped to 1 in 14,952 cases (Aesthet Surg J 2020).

When will I see the result?

Gradually: part of the fat is reabsorbed and the shape settles over the following months.

Can the buttocks be lifted without surgery?

Non-surgical methods do not reposition sagging tissue; when there is real sagging, the assessment defines the right option.
